This usually happens when the primary key of the panel is not being submitted through web message to the system.
You need to make sure, that primary key of the panel should be one of the fields of web message having FORM to be filled. For ex. if primary key of the panel is CUSTOMERID, then it should be captured in one of the fields of web message, then only the contact will get added to the list on getting the identity of the contact.
